By removing the barrier of cost to the end users, it is hoped more patients with long term conditions (LTC’s) such as Asthma, Diabetes, Blood Pressure and Chronic Obstructive Pulmonary Disease (COPD) will be able to take advantage of the t+ system and therefore improve the management of their condition.

Long term conditions currently account for 80% of GP visits, and the prevalence of such conditions in the UK is expected to reach 30% by 2025. The implications of this increase will lead an already stretched National Heath Service to breaking point. It is therefore clear and accepted that the current explosion in chronic diseases such as diabetes, COPD and other respiratory complications requires a new model of healthcare provision. Telemedicine is the only way to prevent healthcare systems around the world from going bankrupt.

Through effective observation of vital signs and symptoms, combined with a healthcare monitoring center, it has been clinically proven that exacerbations and emergency admissions can be dramatically reduced. t+ Medical® offer a solution to monitor a patients symptoms and vital signs through harnessing the power of the patients own mobile phone. The diary is fully interactive with the data entered by a patient being immediately interpreted and then fed back to them in the form of simple graphs, both on their mobile phone-and on a personal web page. There are 5.2 million people in the UK receiving treatment for asthma. Early intervention in asthma can prevent exacerbations that lead to complications and possibly hospital admission.

The new system allows the healthcare professional not only to see individual patient peak flow and inhaler use but also compares and relates the two. t+ Asthma also has a pollution forecast driven by a direct link to the Met Office that gives accurate localised environmental information. This allows both the patient and healthcare professional to spot trends and avoid potential complications.

Asthma sufferers can breathe easy with the new asthma system from t+ medical. More than five million people in the UK receiving treatment for asthma could be helped to manage their condition with the new Personal Advanced Asthma Management System from t+ medical. The mobile phone-based system allows users to effectively take control their condition and may even help save some of the 1,400 deaths from asthma each year.

t+ medical has pioneered a way to help people take control of long term conditions by turning the everyday mobile phone into a personal healthcare management system. The t+ asthma system records peak flow meter measurements, inhaler usage, weather and pollen count and asks a series of questions to help the individual predict the likelihood of an attack and take appropriate preventative measures. To provide further support, the information is also transferred to a personal secure web page that the user can authorise access for healthcare professionals, allowing them to monitor patients’ conditions without having to visit their clinic or surgery. t+ medical also have a t+ Specialist Nurse on-hand to answer any specific medical questions which come through to the customer support team.

“The immediate feedback provided on the patient’s mobile phone provides an easy to use platform for assessing the likelihood of an attack on any given day, meaning that preventative measures can be taken” explains Dr Jeremy Wheeler, GP.
