t+ Blood Pressure

t+blood pressure is a solution for managing patients with hypertension or other conditions requiring the management of blood pressure e.g.  stroke.  Patients can enter an electronic diary via mobile phone or website, recording numeric measurements for weight and blood pressure (systolic and diastolic).

This information is automatically sent to a central server with a website that can be accessed by clinicians or case managers. The clinician can view historical data and charts showing trends, also environmental data such as local pollution levels can be displayed simultaneously.
