t+ COPD is a solution for managing patients with chronic obstructive pulmonary disease (COPD). Patients can enter an electronic diary via mobile phone or website, recording subjective symptoms, medication quantities, and numeric measurements for PEF, FEV and SpO2. This information is automatically sent to a central server with a website that can be accessed by clinicians or case managers. The clinician can view historical data and charts showing trends, also environmental data such as local pollution levels and temperature can be displayed simultaneously and can add notes or send messages to the patient.
