Advanced Telehealth Solution for High Blood Pressure Remote Patient Monitoring

A Patient with high blood pressure has to check their blood pressure several times a day in order to stay healthy.  This can make life difficult to manage amongst a busy lifestyle, potentially resulting in the patient forgetting to take their readings or perhaps not fully understanding their readings at all.  Not only is this unhealthy for the patient but it also means that they must be admitted to the clinic or hospital for medical treatment.

The t+ Blood Pressure telehealth solution turns this situation around by enabling clinicians to monitor their high-blood-pressure patients remotely using only the web and the patient’s mobile phone.  Clinicians can communicate through the system with their patients.

How t+ Medical Blood Pressure Works

Patients are supplied with a mobile phone loaded with our t+ Blood Pressure application, each day the high-blood-pressure patients will submit their daily readings via mobile application or web link.  In the clinic, nurses will have access to a website that supplies them with a list of all their current high-blood-pressure patients.  The list is prioritised automatically by the system based on the severity of the readings submitted.  Nurses can then either call the patient directly to offer advice, text them, email them, or message them using the secure built-in message system.  In addition, there is a configurable alert system designed to text, email, or page the Patient’s designated healthcare professionals should a patient submit readings that need attention.
