The t+ Diabetes telehealth solution turns this situation around by enabling clinicians to monitor their Diabetes patients remotely using only the web and the patient’s mobile phone. The system improves nurse efficiency by allowing the management of more Diabetes patients at once thanks to built-in auto-prioritisation lists, secure one-way-communication functions & built-in titration mechanisms.
How t+ Medical Diabetes Works
Patients can use their own mobile phone or are supplied with a mobile phone loaded with our t+ Diabetes application, each day your Diabetes patients will submit their glucose readings and insulin types/levels. In the clinic, nurses will have access to a portal that supplies them with a list of all their current Diabetes patients. The list is prioritised automatically by the system based on the severity of the readings submitted. Nurses can then either call the patient directly to offer advice, SMS them, email them or message them using the built in secure message system. In addition, there is a configurable alert system designed to text/email designated healthcare professionals should a patient submit readings that suggest an imminent attack. The system even has built in Insulin & Oral titration support.
